Prepare for Your First Therapy Session

Your first visit to PT Northwest will be a little different than your subsequent visits for your therapy sessions. We recommend you arrive 30 minutes before your scheduled time to make sure you have enough time to fill out the registration forms. Your first visit will take about 90 minutes while we ask you some questions and perform an evaluation.

First Visit FAQs

Do I need to bring my prescription form to my first visit?
In general, Yes.  Many insurance companies require you to have a prescription from your physician for rehabilitation services in order for us to provide services to you and receive payment.   

Do I need to bring my insurance cards with me?
Yes. We will make copies of your cards to make sure we are sending bills to the correct address with the correct ID and group numbers.

What do I need to bring for aquatic therapy?
Please bring a swimsuit and towel when you attend aquatic therapy at our Northeast Salem location.

What do I need to wear for my therapy?
Your attire will differ based on your diagnosis. If we are treating your back or extremities for example, it’s best to wear clothing that provides easy access to the area such as a pair of shorts, which we have available at the clinic. If you prefer to wear your own shorts, we ask that they have an elastic waistband. If your therapy will require any form of exercise at our on-site gym, please bring athletic shoes, a t-shirt and either shorts or sweatpants.

Which clinic should I go to?
Our eight clinics allow you to schedule appointments based on proximity and convenience. If your doctor has prescribed aquatic therapy, we can treat you at our Northeast Salem clinic. Contact us to make an appointment at the location that’s most convenient for you.

What do I do if my doctor sends me to PT Northwest, but PT Northwest isn’t a provider for my insurance?
We are a contracted provider for all major insurances in Oregon. Our front office staff will try to verify that your insurance will cover your therapy before you come into the clinic. If you still have questions about your insurance coverage, our billing department will also be happy to help you. The billing department can be reached at 503-540-8701. It is the patient’s responsibility to make sure that they are using a contracted provider. Providers are typically listed in the handbook that your insurance company distributes to you or you can find them at the insurance company’s website. If PT Northwest is unable to see you due to insurance reasons, we will do everything we can to give you some alternative options.

If I am under the age of 18, what do I need to do?
All your requirements are the same, but you will also need to bring your parent or legal guardian to your first visit to sign the release forms. You will be required to provide your Social Security number when you schedule your first visit.
